1928509 Primary Facility 2010-11-17 Routine/Complete Inspection No Violations Noted
1938915 Primary Facility 2010-12-14 Drilling/Alteration No Violations Noted Current Depth 2511'. Est TD = 11469'. Set balance plug at 1013'. Closed and held for 12 hours. On Dec. 12, partial returns were returned (spacer returned with no cement back to surface). Temp survey ran. Determined the need to perf. Shot 8 holes. 1 shot/2ft. 903+ for a total of 8 shots. Attempted to pump into perferations. Pressured up to 1500 psi. Bled off pressure for approx 815 minutes.
1938943 Primary Facility 2010-12-21 Drilling/Alteration No Violations Noted Operater contacted office stating they were able to seal off 13 3/8. Took methane meter (with guage and sniffer) onsite to obtain reading. Reading 0% on all strings. Currently at 3195'.Co. plans on going back in to drill to 5164' (kick off point). Had 2 shows of H20. One at 3072'; the other at 3128'. 2 samples taken and sent to lab to be analyzed. No show of gas.
1944538 Primary Facility 2010-12-20 Drilling/Alteration No Violations Noted 2300' set plug. Squeezed 8 bbls into formation, pressure test held 750 lbs. Well tested good. Commenced drilling.
2199503 Primary Facility 2013-08-13 Routine/Complete Inspection No Violations Noted Inspection is for 'Inactive Well Status" application.
2377350 Primary Facility 2015-06-01 Routine/Complete Inspection No Violations Noted
2579625 Primary Facility 2017-04-03 Routine/Complete Inspection No Violations Noted On April 3, 2017, at 1300 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Scheffler Location in Standing Stone Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The E&S plan, site sign and weekly inspection logs were observed to be on location and available for review. The slopes of the well pad and the topsoil stockpiles were well vegetated. All temporary E&S BMPs were observed to be removed from the site. PCSM has not been addressed at the site.

One well was observed in production phase during the inspection. The well was labeled and no bubbling was observed in the well cellar. One GPU and two production tanks were observed on site. The production tanks were observed to be on secondary containment. Puddles on the pad and liquid within the secondary containment tested low in specific conductivity. No staining was observed on the well pad surface.

The ESCGP-1 permit expire

2677448 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted Well is producing.

Cold/windy at time of inspection.

Vented to production tanks on location.

2854837 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted On March 14, 2019, Department representative Jared Jacobini conducted a routine site inspection at the CHESAPEAKE APPALACHIA LLC SCHEFFLER 1H-6H pad. Jaimie Moran "Environmental Technician" with APEX arrived onsite to conduct E&S inspections. The site sign, drilling permits, weekly E&S reports, and emergency response plan were onsite for review.   The E&S inspection log was reviewed; the last routine inspection was conducted on 03/14/2019. PSCMs have been installed. No erosion issues were observed. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

A total of one producing well is located on this well pad. The well is tagged with an API/well permit number. No bubbling within the cellars was observed. There is one GPU and two steel vertical production fluids tanks onsite. The production fluid tanks were stationed in secondary containment. The GPU was not.

The Department recommends: 1. Applying for a new ESCGP and restoring the site per Restoration Plan. Accordi
