2683020 Primary Facility 2018-01-11 Routine/Complete Inspection No Violations Noted On January 11, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC Bennett Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. A Chesapeake Security employee arrived on site during the inspection.

The site sign, E & S plans and weekly inspection logs were on site and available for review. The last weekly inspection was conducted on 1/8/2018 by Chesapeake. The site was observed to be well vegetated. No erosion issues were observed. The well pad has not been reduced in size, and PCSM has not been addressed.

Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and two GPUs were on site. No staining was observed on the well pad surface. The puddles on the pad were frozen, so conductivity readings were not taken.

No violations were observed during the inspection.

2805517 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:10; the weather was ~40°F, calm, and most of the ground was snow covered. There was a plowed area around the production equipment.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. There were 2 GPUs, 2 produced water tanks, and 2 sand separators onsite and in secondary containment. The containment around the produced water tanks contained some frozen water.

The snow cover prevented a proper assessment of vegetative cover and site stabilization. However, there was a large amount of tall grasses and other vegetation visible above the snow. There was also dense vegetation visible in spots where snow had melted.

Department records indicate that the ESCGP for this site expired on 9/2/15. It also appears that there are no active well permits and no restoration extensions have been acknowledged. It is recommended that if the site is ready for permit termination an NOT be submitted in
