Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1990419 Primary Facility 2011-07-20 Drilling/Alteration No Violations Noted Nomac 8
1996015 Primary Facility 2011-08-16 Routine/Complete Inspection Pending Followup from chk email. Bubbles in cellar, however unable to determine origin as there is too much water in the cellar. Call to Mr Snyder requesting vac truck for further investagation. Problem determining which well is which as there are no markings.
2001694 Primary Facility 2011-08-23 Routine/Complete Inspection Violation(s) Noted Utlizing Model 60 gas meter, 2 % combustible gas between 13 3/8 x 20. 40% LEL. Visible bubbling. SAIC obtained sample.
2078732 Primary Facility 2012-06-21 Routine/Complete Inspection No Violations Noted Inspection conducted on 6/21/12. Pad repairs have been completed since last inspection. Repaired areas have been hydroseeded, but are far from an established 70% vegetated cover. The 1H and 2H appear to be getting close to being brought on line. All major pieces of equipment necessary to place both wells on line are on site and in place. While inspecting the pad, noticed areas on surface of pad that had a whitish appearance. There was one area of standing water on the pad which had this whitish appearance surrounding it. Tested the standing water with an Extech ExStik 2 conductivity meter. The display indicated that the conductivity was above the meters upper range which is listed at 19.99 mS/cm. Water sample # 0257072 taken of the suspect standing water. Of note is the fact that flow back of the wells was completed on June 8th. Also of note is the fact that calcium chloride may have been utilized for dust suppression on this pad.
2082589 Primary Facility 2012-07-11 Administrative/File Review Violation(s) Noted Also cited violation for failure to report a spill incident 91.33 and potential pollution to waters of the Commonwealth under CSL 402
2117648 Primary Facility 2012-08-16 Routine/Complete Inspection No Violations Noted Well hooked up for production, however currently shut-in. 0 psi on 13 3/8 and 9 5/8 annuli. 1750.1 psi on production tubing. 2 seperators, 2 condensate tanks.
