2705322 Primary Facility 2018-03-05 Routine/Complete Inspection No Violations Noted On March 5, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Roeber Location in Wyalusing Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was stabilized with a combination of rock and vegetation. The entire cut slope was stabilized with rock. No erosion or sedimentation issues were observed. The well pad has not been reduced in size. PCSM has been addressed at the site with the installation of two infiltration areas. The Department observed an 18 inch culvert discharging outside of the infiltration berm. According to the onsite plans, the 18 inch culvert is intended to discharge into the basin. The Department recommends installing the infiltration basin as per the approved ESCGP-2.

One well was in production during the inspection. The well was tagged and no bubbling was observed in the well cellar. Two

2823788 Primary Facility 2019-01-08 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 10:57; the weather was ~35°F and calm. The pad surface had some spots of ice but was mainly dry.

There was 1 properly tagged Marcellus well staged for production. There were 3 well cellars filled with ice; no bubbling was observed. There was 1 GPU and sand separator and 2 produced water tanks onsite and in secondary containment.

Areas of vegetation were greater than 70% cover but much of the area is stabilized with rock. No E&S issues were noted at the time of inspection. It appears the ESCGP for this pad expires on 6/16/21.
