2844740 Primary Facility 2019-02-22 Routine/Complete Inspection No Violations Noted Arrived onsite for a complaint inspection at 10:25; the weather was ~40°F and overcast. The gate was open when I arrived.

The Department received a complaint for excessive water/ice in a homeowner's yard near the pad on 2/19/19. I walked with the homeowner in order to observe the areas on concern. The homeowner believed the water was running from the northern end of the pad and from around Infiltration Berm A. I then proceeded on my own to inspect the pad a surrounding area.

Several areas along the access road/Ferris Wheel Farm Rd. Showed signs of erosion in the roadside ditch (Photos #2-4). Additional evidence of erosion was noted near the outlet of the culvert under the access road near the pad entrance. Erosion was also observed above the SW corner of the pad (Photo #5).

There appeared to be water flowing from the NE corner of the pad and forming a gully (Photo #8, 13, 14). Upon discussion with Department engineers it was suggested that this may indicated that water is not flowing to the low spot of th
