Temporary erosion and sediment control BMPS have been removed. Vegetative cover of the site is greater than 70%. No signs of accelerated erosion or sedimentation were observed. No areas of staining or other indicators of spills were observed on the well pad.

An extension of time for site restoration was approved by the Department on January 19, 2016. The post-construction stormwater management BMPs required by the restoration extension approval have been installed.

No violations were cited during this inspection.

2679994 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted One well in production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted.
2796522 Primary Facility 2018-10-26 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. There was very little water on the pad surface and in secondary containment.

There was 1 properly tagged Marcellus well staged for production. There was 1 GPU and 2 produced water tanks onsite and in secondary containment. The produced water tanks had moderate corrosion.

The cut slope of the pad had 4 areas where slips occurred (Photos #5, 6, 7). According to the the E&S inspection log these slips occurred in January 2018 and have since been addressed. These areas and most of the fill slope were still straw covered and had sparse vegetation. At the time of inspection no evidence of accelerated erosion was observed.

It is recommended that these areas be reseeded as weather permits, if necessary, to ensure proper vegetative cover. According to Department records the ESCGP expired on 1/2/16 and a 2-year restoration extension was accepted on 2/1/18.
