2485462 | Primary Facility | 2016-05-18 | Routine/Complete Inspection | No Violations Noted | Vegetative cover on site greater than 70% and consisting of mainly perennial plant species. Temporary erosion and sediment control BMPs have been removed, with the exception of areas to be disturbed during installation of post-construction stormwater management (PCSM) measures. No evidence of accelerated erosion or sedimentation was observed.
A 2-year extension of time for site restoration was approved by the Department on 01/20/2016. As a condition of this approval, PCSM features must be installed by 10/20/2016. At the time of inspection preparation for construction of such features was underway. No violations were cited during this inspection. |

2736716 | Primary Facility | 2018-06-01 | Follow-up Inspection | No Violations Noted | Arrived at 8:00 for the conclusion of 72-hour pressure buildup tests on the 3 producing wells. Met with: Giovanni Dicupillo (Chesapeake) and Shana Erway and Sean (Leidos) to split isotopic gas samples from the 13-3/8" x 9-5/8" and 9-5/8" x 5-1/2" annuli of each well. Annuli had been shut-in on Tuesday, May29 at: 2H - 7:10, 5H - 7:15, 6H - 7:20.

During the 6H sampling, a group of bubbles were seen, approximately 3 times, with an interval of 5 to 10 minutes between bubble episodes. Over the next hour, no bubbling was observed. |
2837309 | Primary Facility | 2019-02-06 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection at 13:20; the weather was ~40°F, breezy, and raining. The gate was open when I arrived.
There were 3 properly tagged Marcellus wells staged for production. The cellars were filled with water; no bubbling was observed. There were 3 GPUs and sand separators onsite, and 2 produced water tanks onsite and in secondary containment. Overall, the site was greater than 70% vegetative cover. There appeared to be 2 infiltration areas constructed near the northeast and southeast corners of the pad. There were 2 very small areas of bare soil on the containment berm (Photos #2, 3). The Department recommends removing the oak stakes as they are no longer needed (Photos #6, 7). No violations noted. |
