Pressure readings were taken from on-site Scada gauges and annuli were vented to the production tanks. All three cellars were full of ice.
2506426 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted On July 27, 2016 at 1130 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Dziuba Well Pad Location in Tuscarora Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Chesapeake did not have a representative present during the inspection.

The site ID sign, E&S plan and weekly inspection reports were all on site and available for review during the inspection. Chesapeake was granted a 2-year pad restoration extension on 1/20/2016. As part of the extension PCSM BMPs had to be installed at site. The Department observed that three infiltration berms were installed at the site. Compost filter sock was installed around the PCSM BMPs and appeared to be functioning properly. There were no E&S issues observed. The PCSM BMPs were observed to be approximately 60% vegetated.

There were two wells that were in production phase during the inspection. No bubbling was observed in the well cellars. The Department

2674532 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted On December 18, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Dziuba Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. There was site was covered in approximately one inch of snow. The Department could not make an accurate assessment of vegetative cover, but vegetation appeared to be adequate. No erosion issues were observed. PCSM has been addressed at the site with the installation of infiltration berms. The well pad has not been reduced in size.

Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks were in secondary containment. Two GPUs were also on site. The puddles on the pad were observed to be frozen so no conductivity readings were taken. No staining was observed on the well pad surface.

2781609 Primary Facility 2018-09-28 Site Restoration No Violations Noted Chesapeake Appalachia submitted a Notice of Termination (NOT) for the Dziuba pad covered under ESX 10-015-0354. The NOT was received by the Department on 9/21/18.

Kyle Stevens was onsite working on GPU/pipeline.

There are 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed and the conductivity was 86.4 µS/cm. There are 2 each GPUs and sand separators onsite and in secondary containment.

Overall the site is >70% perennial, vegetative cover. There are 3 infiltration berms onsite as PCSM BMPs. They appear to have been constructed as designed and functioning correctly.

There was some rusting of the produced water tanks. Kyle believed they were on a list to be replaced with fiberglass tanks in 2019.

No violations noted.

The Department recommends acknowledging termination of coverage under ESX 10-015-0354.
