Vegetative cover on site >70% and consists mainly of perennial plant species. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

A 2-year extension of time for site restoration was approved by the Department on 02/03/2016. That approval requires that post-construction storm water management (PCSM) features be installed by 05/03/2016. At the time of inspection, the PCSM features had not yet been installed. However, staking was in place at the locations where such features are to be installed.

No violations were cited during this inspection.

2814816 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:02; the weather was ~30°F and calm. The pad had several pieces of farm equipment being stored on the eastern edge.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with ice; no bubbling was observed. There were 2 GPUs and sand separators onsite and in secondary containment.

The cut and fill sides of the pad were stabilized with rock. The remaining 2 sides were more gently sloped and were well vegetated. Overall, the area was greater than 70% vegetative cover. No E&S issues were observed.

The Department recommends repairing or replacing produced water tanks as needed.

No new violations noted as a result of this inspection.
