2613330 Primary Facility 2017-07-07 Routine/Complete Inspection No Violations Noted On July 7, 2017, at 1145 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Fisher Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was observed to be well vegetated and no erosion issues were observed. PCSM has not been addressed at the site.

Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two GPUs and two production tanks were also on site. The production tanks were on secondary containment. Puddles on the pad surface were low in specific conductivity. No staining was observed on the well pad surface.

The ESCGP-1 permit has expired. The well pad has not been reduced in size, and PCSM has not been addressed. The Department recommends repermitting the site to address PCSM or rest

2783066 Primary Facility 2018-09-12 Incident- Response to Accident or Event No Violations Noted I arrived at the site 12:12 PM in response to a spill of an estimated spill of (30) barrels of production fluid to the secondary containment unit for the production fluid storage tanks. The weather conditions are cloudy skies, near calm winds, with moderate to heavy rain falling, and temperatures around 70 degrees Fahrenheit. There was no activity or personnel at the site. CHK submitted a Deficient Tank Inspection Notification (Unconventional Wells) 78.57(i) for well Permit # 015-21782 to the Department on 9/6/14 at 13:14 hours. The notification and deficient tank inspection is associated with the Fisher Bra 2H well. CHK provided the following information in the tank inspection notification. The tank contents consists of produced water, with the capacity of the tank being (400) barrels. A small leak on sidewall of steel tank #1. The estimated volume lost was (30) barrels.
