2482419 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70% and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The Department's records indicate that no current well permits exist for the site and no extension request for site restoration has been submitted or approved. In accordance with the information you provided to the Department by letter dated September 21, 2015, this site has been identified as a site for which a restoration extension request would be submitted. This information was submitted in response to the Department's June 26, 2015 letter requesting your plans to comply with Section 3216 of the 2012 Oil and Gas Act and 25 Pa. Code Section 78.65, which relate to site restoration. You are reminded that such an extension request should be submitted as soon as possible.

No voilations were cited during this inspection.

2695386 Primary Facility 2018-02-16 Drilling/Alteration No Violations Noted Frack Tech Corp is conducting stimulation activities on the 4H at this time. All materials within primary and secondary containment. Rig matting/timber mats installed on the primary containment. No leaks detected. No violations noted.
2703363 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted I arrived at the site at 10:25. The weather conditions were partly cloudy skies, breezy and mild with temperatures in the mid to upper 50's. I was accompanied by Sean VanFleet, DEP O&G EPS of DEP NCRO in Williamsport, PA. The main intent of the inspection was to view the process of treating flowback and production fluid generated from various CHK wells and water transferred to this pad into an above ground storage structure from a nearby CHK well development impoundment, with chlorine dioxide. On 1/25/18 the Department approved a Request for Approval of Alternative Waste Management Practices, (OG-71) Form A application for residual waste processing. For additional details on the process see photos, comments in the findings section for on-site processing/waste control and disposal in this inspection report, the application in EFACTs Authorization ID# 1214787, and comments below in the narrative. We met with Nick Krut, Eric Hottenstein, Jim Baker and Darin Chaffee of CHK on the Cuthbertson Pad. They provided u
2723766 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted I arrived at the Cuthbertson Bra pad at 13:35. The weather conditions are cloudy skies, cold with light to moderate winds and temperatures in the low 40's. There was no activity or personnel at the site. The Cuthbertson Bra 2H and Walker Bra 4H wells are in production. I recorded elevated specific conductance (conductivity) of water in the containment and pad surface in the center of the pad near the wells. I observed trash and debris scattered around the perimeter of the site, which includes the pad surface site perimeter stormwater ditch and site fill slope. See photos. I departed from the site at 14:00.
2837263 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:41; the weather was ~36°F, overcast, and calm. The gate was open when I arrived.

There was a well tender at the Cuthbertson pad, however he was on the phone. There were 2 properly tagged Marcellus wells staged for production. There were 6 well cellars onsite. The concrete area around the wells was underwater. There were 2 GPUs, sand separators, and produced water tanks onsite and in secondary containment. The areas around the pad were greater than 70% vegetative cover. No E&S issues were observed.

The Walker pad had 1 properly tagged well staged for production. There were 2 cellars filled with water and 1 cellar that appeared to be backfilled. No bubbling was observed. There was 1 GPU, 1 sand separator, and 2 produced water tanks in secondary containment. The areas around the pad were greater than 70% vegetative cover. No E&S issues were observed.

The Department recommends submitting an NOT and restoration report if the pads meet restoration criteria. Other
