2800020 Primary Facility 2018-11-02 Site Restoration No Violations Noted A Notice of Termination (NOT) was received by the department on October 23, 2018 which prompted this inspection.

One well is staged for production with one GPU and sand separator, and one double-walled fiberglass produced water tank. Produced water tank is in good condition overall.

Post Construction Stormwater Management (PCSM) is handled by three infiltration berms and a series of rock-lined ditches. These structures appear to be functioning as designed. Water is currently present in infiltration areas #2 and #3. The pad has been permitted to remain at full size. Side slopes, infiltration areas, and the area surrounding the access road within the LOD are well-vegetated with greater than 70% vegetation.

No E&S issues noted on or surrounding the pad.

A permitted stream crossing exists along the access road (Permit GP-07-0829-11-14). Here, an unnamed tributary to Towanda Creek crosses beneath the access road through a 30' dual 9'x4' concrete box culvert stream crossing. This concrete culvert has been co
