2626392 Primary Facility 2017-07-12 Site Restoration No Violations Noted On July 12, 2017, the Department conducted a restoration inspection at the Chesapeake Appalachia, LLC, DJ Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake. A Notice of Termination (NOT) was received by the Department on July 10, 2017.

The site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(i) Permanent Stabilization. The slopes of the pad were well vegetated. All temporary E & S controls have been removed from the site. No erosion issues were observed.

Two infiltration berms were installed on site. The berms were installed as per the approved ESCGP-2, but the berms did not appear to be receiving any stormwater from the pad. The submitted As-Built drawings did not completely reflect the approved ESCGP-2 plan that was approved. In the approved plan, the containment berm was to be reconstructed near the production tanks. 2851090 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted On March 07, 2019 Department representative Jared Jacobini conducted a routine site inspection at Chesapeake Appalachia LLC DJ Location pad. The site sign, drilling permits, monthly tank maintenance log, and emergency response plan were onsite for review. The tank inspection log was reviewed; the last routine inspection was conducted on 02/27/19. Infiltration berms, compost filter socks and rock lined channels are functioning properly. No erosion issues were observed. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

A total of 2 producing wells located on this well pad and tagged with API/well permit numbers. No bubbling within the cellars was observed as they contained frozen snow and ice. There is one GPU and one double walled fiberglass vertical production fluid tank onsite. The GPU did not appear to have internal secondary containment. A Methanol injection unit was present and within containment.
