Site well vegetated with greater than 70% perennial vegetation. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. Electrical conductivity was not screened due to absence of puddles on pad.

The most recent well permit for this site expired on 04/30/2016, and the Erosion and Sediment Control Permit expires on 07/18/2016. According to the Department's records, no request for a 2-year extension of time to restore the site has been filed, and no new well permit is pending. In order to remain in compliance with Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to site restoration) a new well permit should be obtained or an extension of time for site restoration should be requested.

There was 1 properly tagged Marcellus well staged for production. There was a total of 3 well cellars filled with water; no bubbling was observed. There was 1 GPU and 1 sand separator onsite and in secondary containment. The containment around the produced water tanks had a small amount of ice and water in it.

According to Department records the ESCGP for this pad expired on 7/18/16. It is recommended that, if the pad meets restoration criteria, an NOT be submitted for review during the spring or summer.

The Department recommends replacing or repairing the produced water tanks as necessary to ensure their integrity.

No violations were noted at the time of inspection.
