2688129 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted Four wells staged for production phase. Four each sand and water separators. No leaks detected from production equipment. No violations noted.
2852218 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:04; the weather was ~45°F, clear, and calm. The gate was open when I arrived and a well tender was in his truck. He waved as I approached but appeared to be busy so I didn't interrupt.

There were 4 properly tagged Marcellus wells staged for production. There were 4 GPUs and sand separators and 1 produced water tank onsite and not in secondary containment. Areas around the pad were sufficiently vegetated and no E&S issues were noted.

It is recommended that if the pad meets restoration criteria, PCSM has been addressed, and it is appropriate to do so an NOT and restoration report be submitted as the weather improves.

No violations noted.
