2646855 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted On October 4, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Merryall Location in Wyalusing Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion or sedimentation issues were observed. PCSM has not been addressed at the site.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and four GPUs were also observed on site. No puddles or staining were observed on the well pad surface.

No violations were observed during the inspection.

2786278 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. The site was >70% perennial, vegetative cover. No E&S issues were observed. All PCSM BMPs appeared to be installed and functioning as intended.

There are 4 properly tagged Marcellus wells staged for production. There are 4 GPUs and 2 produced water tanks also onsite and in secondary containment. There was moderate rust on the produced water tanks (Photo #4). It is recommended that storage units and equipment be inspected and maintained or repaired as needed.

It appears that the ESCGP for this site expired on 9/1/16. It is recommended that if earth moving activities are completed and the site meets restoration guidelines a notice of termination be submitted to the Department.
