2576052 Primary Facility 2017-03-28 Routine/Complete Inspection No Violations Noted On March 23, 2017 at 1045 hrs., the department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Stone location, in Tuscarora Township, Bradford County. The department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for Chesapeake.

The E&S plan, site sign, and weekly inspection logs were on site and available for review. The slopes of the well pad and the well pad surface were covered with snow. PCSM has not been addressed at the site. No erosion issues were observed.

Two wells were observed in production phase during the inspection. Both wells were labeled and no bubbling was observed in the well cellars. Two production tanks and two GPUs were on site. The production tanks were on secondary containment. Puddles observed on the pad surface were frozen. No conductivity readings were taken.

A two year restoration extension was approved by the Department on October 12, 2016. PCSM plans were submitted and approved as a part of the restoration

2699070 Primary Facility 2018-02-26 Routine/Complete Inspection No Violations Noted On February 26, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Stone Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last weekly inspection occurred on 2/22/2018. The site was mostly well vegetated. The newly installed infiltration berm had a few areas that needed to be reseeded, however vegetation was observed to be well established. The well pad has not been reduced in size, but PCSM has been addressed at the site. The Department observed four wooden stakes in the containment berm surrounding the well pad that seemed to delineate the proposed berm break that was identified in the E & S plan. The berm break was not observed to be installed in this area. The Department recommends installing the berm break, or modifying the plans and submitting them to the Department for review.
