2628667 Primary Facility 2017-08-18 Site Restoration No Violations Noted On August 18, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Felter Location in Wyalusing Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A Chesapeake worker was on site during the inspection. A Notice of Termination (NOT) was received by the Department on 8/17/2017.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Generally the site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. No erosion or sedimentation issues were observed. The well pad has not been reduced in size, but a vegetated filter strip was observed to be installed to address PCSM.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and four GPUs were also on site. Puddles on the pad surface were low in specific conductivity. No staining was observed on

2728500 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted Fours wells staged for production phase. Four each water separators. No leaks detected from production equipment. A puddle near the production tanks returned a value of 0.62 mS/cm. No violations noted.

Last E&S inspection done on 4/30/2018.
