The well permits for this site expired on 01/11/2013, and the Erosion and Sediment Control Permit expires on 11/06/2016. Title 58 Pa. C.S. § 3216 (relating to restoration)(2012 Oil and Gas Act) requires that well sites be restored within 9 months of completion of drilling. In order to remain in compliance with the Department's restoration requirements, the site should be restored or a 2-year extension of time for site restoration should be requested.

Vegetative cover on site >70% and consisting mainly of perennial plant species. No evidence of accelerated erosion or sedimentation observed.

According to the Department of Environmental Protection's records, no new well permit is pending and no request for a 2-year extension of time for site restoration has been requested. In order to remain in compliance with the regulations pertaining to site restoration ( Section 3216 of the 2012 Oila nd gas Act and 25 Pa. Code Section 78a.65) a new well permit should be obtained or an extension of time for site restoration should be requested.

No violation were cited durign this inspection.

2686421 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted Two wells staged for production phase. Two each sand and water separators. No leaks detected from production equipment. No violations noted.
2808163 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:45; the weather was ~35°F, calm, and lightly snowing. The pad and surrounding area were bare with the exception of some snow cover near the edge of the woods.

There were 2 properly tagged Marcellus wells staged for production. There was a total of 3 well cellars onsite and filled with water; no bubbling was observed. There were 2 GPUs and 2 sand separators onsite and in secondary containment. The containment around the produced water tanks had a small amount of ice.

Along the access road near the pad there was an area that appeared to lack vegetation (Photo #2). This area showed the potential for a small amount of erosion to develop. It is recommended that areas around the pad be stabilized as needed and as weather permits.

According to Department records the ESCGP for this pad expired on 11/6/16. It is recommended that, if the pad meets restoration criteria, an NOT be submitted for review after the winter.
