2642588 Primary Facility 2017-09-29 Incident- Response to Accident or Event No Violations Noted I arrived at the site in response to a deficient tank notice received on Thursday 9/28 stating that 11 barrels of produced water had been released from a small hole in the tank, and the release was completely captured by the secondary containment around the tanks. I noted the small 1/2-3/4" hole at approximately 10' up the tank. No fluid appeared to have gone beyond the secondary containment.
2845029 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted On February 21, 2019, Department representative Brad Kline conducted a routine site inspection at Chesapeake Energy Jag Pad. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There is a total of 1 producing well located on this well pad. The well is tagged with API/well permit numbers. No bubbling within the cellar was observed. There is 1 GPU with a sand separator and 1 vertical production fluids tank onsite. Both GPU and production fluid tank were not stationed in secondary containment.
