Vegetative cover on site is generally greater than 70%, with some bare spots and rocky areas present. Temporary erosion and sediment control BMPs are still in place. No evidence of accelerated erosion or sedimentation was observed.

A restoration extension request was approved by the Department on 12/07/2015. Post-construction Stormwater Management BMPs have already been implemented.

No violations were cited during this inspection.

2674656 Primary Facility 2017-12-26 Routine/Complete Inspection No Violations Noted One well in production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted.
2792769 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection; weather was ~39 °F, calm, overcast. Some shallow puddles were on the pad, the deepest of which had a conductivity of 315.0 µS/cm.

There was 1 properly tagged Marcellus well staged for production. This well cellar was nearly empty while the 2 other cellars onsite were full of water. No bubbling was observed in any of the cellars. There was 1 GPU, 1 sand separator, and 2 produced water tanks onsite and in secondary containment.

The E&S and PCSM plans onsite depicted compost filter sock sediment traps. At the time of inspection the filter sock had been removed from the site and redlined off of the plans.

It appears that the E&S permit (ESX11-015-0311) for this site has expired on 1/8/17. It is recommended that if PCSM has been constructed as needed and the site meets restoration criteria an NOT be submitted to the department for review.
