2576054 Primary Facility 2017-03-28 Routine/Complete Inspection No Violations Noted On March 23, 2017, at 1155 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Wildonger Location in Wyalusing Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The E&S plan, site sign and weekly inspection logs were on site and available for review. The slopes of the well pad were partially covered in snow. The areas that were exposed were well vegetated. No erosion issues were observed. Sediment traps were installed and partially filled with water during the inspection.

Two wells were in production phase during the inspection. Both wells were labeled and no bubbling was observed in the well cellar. Two production tanks and two GPUs were observed on site. The production tanks were observed to be on secondary containment. Puddles on the well pad were low in specific conductivity.

The ESCGP-1 expired on 12/19/2016. According to Department records, there are no active well permits fo

2837240 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:55; the weather was ~35°F, calm, and overcast. The gate was open when I arrived. I talked with the well tender onsite, Adam. He mentioned that everything had been going well and he was taking care of some routine things. He also stated that the steel produced water tanks were scheduled to be replaced with fiberglass this year.

There were 2 properly tagged Marcellus well staged for production. The 2H cellar and an empty cellar were filled with ice/water; no bubbling was observed. The 3H cellar was pumped out. There were 2 GPUs and sand separators onsite, and 2 produced water tanks onsite and in secondary containment.

Vegetated areas around the site were greater than 70% cover while the steeper slopes were stabilized with rock. The containment berm was tree covered. There appeared to be an infiltration area constructed of the southwestern edge of the pad.

The Department recommends submitting an NOT and restoration report in the spring or summer if the site meets
