According to the Department's records, no new well permit is pending and no request for a 2-year extension of time for site restoration has been requested. In order to remain in compliance with the Department's restoration requirements, a new well permit should be obtained or an extension of time for site restoration should be requested.

Code Section 78a.65) a new well permit should be obtained or an extension of time for site restoration should be requested.
