Vegetative cover on site is generally greater than 70% perennial vegetation, with the exception being rock areas on the fill slope of the pad and sparsely vegetated areas on the pad cut slopes. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

According to the Department's records, no current well permit exists for this site and the Erosion and Sediment Control General Permit expires on 12/21/2016. No request for a 2-year extension of time to restore the site has been filed, and no new well permit is pending. In order to remain in compliance with Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to site restoration) a new well permit should be obtained or an extension of time for site restoration should be requested.

No violations were cited during this inspection.

The Department's records indicate that no current well permits exist for the site and no extension request for site restoration has been submitted or approved. In September 2015, you indicated that a restoration extension request would be submitted. To date no restoration extension request has been filed with the Department. In order to maintain compliance with the Department's regulations regarding site restorations ( Section 3216 of the 2012 Oil and Gas Act; and Title 25 Pa. Code Section 78a.65) a restoration extension request or application for a new drilling permit should be filed.

2793464 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection; weather was ~41°F, overcast, calm. There were some puddles on the pad surface but they were too shallow for conductivity screening.

There was 1 properly tagged Marcellus well staged for production and 2 additional well cellars. All 3 cellars contained water but no bubbling was observed. There was 1 GPU, 1 sand separator, and 2 produced water tanks onsite and in secondary containment. The produced water tanks had corrosion around the bottom and sides of the tanks (Photos #3, 4, 6).

Overall the site is >70% vegetative cover. However, some portions of the containment berm around the pad have sparse vegetation. These areas were somewhat rocky and at the time of inspection no E&S issues were observed.

It appears that the E&S permit (ESX11-015-0301) has expired on 12/21/16 and no restoration extension has been submitted. It is recommended that an attempt be made to reseed sparsely vegetated areas to ensure stabilization as weather permits. If the site then meets restorat
