2829904 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted Chesapeake Appalachia submitted a Well Site Restoration Report (WSRR) for the Circle H pad. The WSRR was received by the Department on 1/14/19. Arrived onsite at 11:26; the weather was ~20°F and calm. The gate was open when I arrived, however the access road also joins another pad.

There was 1 properly tagged Marcellus well staged for production. There were 6 well cellars filled with ice. There was 1 GPU and sand separator, and 2 produced water tanks onsite and in secondary containment.

Much of the site was sufficiently vegetated. However, some areas of the containment berm lacked vegetation and showed the potential to develop accelerated erosion issues (Photos #1, 4, 5). At the time of inspection small areas scattered around the site had sparse vegetation. A large area near infiltration area #1 on the northern side of the pad contained a large amount of mossy and in some cases sparsely vegetated ground (Photo #2, 6).

It is recommended that the areas mentioned above be reseeded with the approved seed mixt
