2485440 Primary Facility 2016-05-17 Routine/Complete Inspection No Violations Noted Portions of the site have less than 70% vegetative cover. Recent pipeline construction has contributed to this. Temporary erosion and sediment control BMPs are in place. No evidence of accelerated erosion or sedimentation woas observed.

A 2-year extension of time for site restoration was approved by the Department on 12/08/2015. As a condition of the extension, post-construction stormwater management (PCSM) measures must be implemented by 03/08/2016, unless additional time is granted, in writing, by the Department. At the time of this inspection, the PCSM measures have not been implemented, and no extension of the deadline to implement these measures was found in the Department's records. In order to remain in compliance with Title 58 Pa. C.S. § 3216 (relating to site restoration)(2012 Oil and Gas Act) and 2-year extension of time granted by the Department, the PCSM measures should be implemented promptly or additional time to do so should be requested in writing.

No violations were cited during this ins

2565220 Primary Facility 2017-02-27 Routine/Complete Inspection No Violations Noted No violations cited.
2814343 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:45; the weather was ~30°F and calm.

There was 1 properly tagged Marcellus well staged for production with a cellar containing ice. The remaining 2 cellars were covered. There was 1 GPU, 1 sand separator and 2 produced water tanks onsite and in secondary containment.

Most areas around the pad were greater than 70% vegetative cover with the exception of steeper rock lined slopes. The site appeared stable and no E&S issues were observed.

At the access road entrance the consumptive water use permit was posted, however the writing was faded and illegible (Photo #1). All other signs were okay.

No violations were noted at the time of inspection.
