2278480 Primary Facility 2014-06-09 Incident- Response to Accident or Event Pending Arrived at the site at 11:50. Chesapeake Energy was in the process of setting up production equipment. I was accompanied by Sean VanFleet, PADEP Oil and Gas Environmental Protection Specialist out of PADEP NCRO. Eric Hottenstein of Chesapeake was at the site during the inspection. Mr. Hottenstein contacted the Department on 6/2/14 to report staining discovered on the ground surface of the Well Pad underneath the containment liner set in place during completions operations. A tear was noted at the seam of the liner where the blender utilized during hydraulic fracturing operations was formerly staged. An unknown volume of hydraulic fracturing fluid flowed through the tear in the liner and spread on the ground surface of the Pad beneath the liner. Mr. Hottenstein stated that white crystallization was observed on the ground surface of the Pad where the fluid flowed through and spread under the liner.

A review of the remediation summary report submitted by Chesapeake indicates that the remediation was adequate. No further action is required of Chesapeake at this time.

On 2/6/15, EC&B forwarded an Approval for a Remedial Action Completion Report to my attention. The report was also submitted to Chesapeake. The report indicates no technical deficiencies were noted with the Alternative Remediation provision of the department policy document 800-5000-01, addressing Spills and releases at Oil and Gas Well Sites or Access Road.

There was 1 properly tagged Marcellus well staged for production. There was a total of 3 cellars onsite filled with ice and water; no bubbling was observed. There were 2 produced water tanks, 1 GPU, and 1 sand separator onsite.

Overall, the area was well vegetated and no E&S issues were observed.
