2176543 Primary Facility 2013-06-19 Follow-up Inspection No Violations Noted Bubbling in surf/cond annulus reported by Chesapeake.Also pressure on 9 x 13 annulus. Surf/cond cap has been installed. Checked the 2 annuli w/ gas meter. 20 X 13 3/8" = 0% methane, and 13 3/8 X 9 5/8" = 100% methane and > 100% LEL on combustible.

2355138 Primary Facility 2015-03-19 Routine/Complete Inspection No Violations Noted Inspected site at 11:52 on 03/19/2015. No Chesapeake representatives were present. Sunny, breezy, ~40°F.
2482395 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted Vegetative cover on site is greater than 70% and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

A 2-year extension of time for site restoration was approved by the Department on 12/07/2015.

No violations were cited during this inspection.

2565495 Primary Facility 2017-02-28 Routine/Complete Inspection No Violations Noted No violations cited.
2703480 Primary Facility 2018-03-06 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 12:40. Weather: sunny. One well on a three-slot set up. Well appears to be TD'ed but not fracc'ed. Heating unit on site and in operation heating well assembly. Chesapeake explained that the heating was to prepare the well assembly for seals work. The seals work in turn was in preparation for fracc'ing later in the summer. No new violations were observed during my inspection. The result of this inspection is, OUTST, due to previously cited violations that are enumerated in this report.

    • The violation code for 25 Pa. Code § 78.85(a)(5) was updated on May 6, 2019 because this violation code was not available in eFACTS when this inspection record was created. This violation was previously listed as §78.85, which was the closest violation code available in eFACTS to the actual violation at the time. Although this is an unconventional gas well currently subject to the Rules and Regulations of 25 Pa. Code §78a, the violation was originally cited in 2014 and the §78a regulations were not effective
2816379 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 10:31; the weather was ~35°F and windy.

There was 1 properly tagged Marcellus well staged for production. The 3 well cellars were partially filled with water; no bubbling was observed. There were 2 GPUs, 2 sand separators, and 1 produced water tank onsite.

The bulk of the area was greater than 70% vegetative cover. There were 3 areas of the pad containment berm that were lacking proper vegetation. The first area was straw covered and had some growth but may need to be reseeded in the spring (Photo #1). The other areas were bare dirt and showed some signs of erosion of the inside of the containment berm (Photos #2, 3).

No violations were noted at the time of inspection.
