2730374 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted One well staged for production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted.
2855928 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:59; the weather was ~35°F, overcast, and calm. The gate was open when I arrived.

There was 1 properly tagged Marcellus well staged for production. There was a total of 3 cellars onsite and filled with ice. Also onsite were 1 GPU, 1 sand trap, and 2 produced water tanks. There was an insulating blanket around the pipes of the produced water tanks that had started to loosen.

Multiple areas along the access road and near the pad were disturbed and lacked proper vegetation. Some of these areas were also becoming deeply rutted due to vehicles or equipment use. It appeared that these disturbances may be related to the landowner logging nearby as noted in the onsite inspection logs. The pad itself appeared to be stable.

It is recommended that an attempt be made to stabilize and restore the disturbed areas within the LOD. It is also recommended that if the site is stabilized, temporary E&S controls have been removed, and it is appropriate to do so an NOT be submitte
