2257418 Primary Facility 2014-03-25 Routine/Complete Inspection No Violations Noted Arrived 12:18 on overcast, cold day. Crew were flowing back. Observed sudden emission of vapor and gas from open circular port on tanks in line for flowing back. Crew were very active on site. The sudden emission occurred 3 times over my ten minutes on site. The vapor appeared to be blown by the wind toward the pad entrance. I did not conduct a further inspection. It is worth remarking that the emission could be a source of contamination to the well pad, the significance of which would depend on how often it occurred. I did not observe any other issues on the location, but my inspection was too brief and limited to provide a useful assessment of this.
2461497 Primary Facility 2016-03-04 Routine/Complete Inspection No Violations Noted Vegetative cover on site is generally greater than 70%, with some bare spots and rocky areas present. Ttemporary erosion and sediment control BMPs are still in place. No evidence of accelerated erosion or sedimentation was observed. Electrical conductance of puddles on the well pad ranged from 320 uS/cm to 450 uS/cm.

A restoration extension request was approved by the Department on 09/16/2015. Post-construction Stormwater Management BMPs have already been implemented.

No violations were cited during this inspection.
