Vegetative cover on site is greater than 70%. No evidence of accelerated erosion or sedimentation was observed . The electrical conductivity of puddles on the well pad was not screened due to the absence of puddles.

To date, no request for a 2-year extension for site restoration was received by the Department. In order to remain in compliance with the Department's regulation pertaining to site restoration (Title 58 Pa. C.S. § 3216), a new well permit or a request for a 2-year extension of time should be submitted.

No violations were cited during this inspection.

2561769 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. Erosion and sediment control BMPs in place along fill slope of pad. Vegetative cover >70 percent. No evidence of accelerated erosion or sedimentation observed.

Containment around produced water tanks about 25% full of snow and ice. The containment should be emptied when conditions allow.

According to the Department of Environmental Protection's records, no new well permit is active or pending for this location and no request for a 2-year extension of time for site restoration has been received. In order to remain in compliance with the DEP's regulations regarding site restoration a new well permit should be obtained or an extension of time requested.

Finan Bra 5H, 015-222698. Top digital gauge: 1029.7 psi, 13-3/8" x 9-5/8" digital, plumbed into produced water tanks: -1.626 psi, 9-5/8" x 5-1/2": -1 psi, 5-1/2" x tubing: 28.62 psi., water-filled cellar no bubbles.

2796351 Primary Facility 2018-10-26 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. There was very little water on the pad surface and only a small amount in secondary containment.

There was 1 properly tagged Marcellus well staged for production. A total of 3 well cellars were onsite and filled with water; no bubbling was observed. There was 1 GPU and 2 produced water tanks onsite and in secondary containment. The produced water tanks had a small amount of corrosion.

At the time of inspection PCSM appeared to be constructed as depicted on PCSM/SR plans and functioning correctly. Overall, the site appeared stable. The areas of vegetation were >70% but the majority of the sloped areas were rock lined. Some compost filter sock (CFS) remained along the access road and near an infiltration area (Photos #1, 2, 4). No E&S issues were noted.

According to Department records the ESCGP for this site expired on 1/22/18 and no restoration extension requests have been submitted. It is recommended that an NOT be submitted for review if all PCSM has been address

2806568 Primary Facility 2018-11-26 Site Restoration No Violations Noted Chesapeake submitted a Well Site Restoration Report (WSRR) for the Finan pad. The WSRR was received by the Department on 11/21/18.

Arrived onsite 10:20; it was raining, very windy, and ~40°F. The pad surface was ice except for a path plowed around storage and production equipment.

There was 1 properly tagged Marcellus well staged for production. The well cellars were filled with water; no bubbling was observed. There was 1 GPU and 2 produced water tanks onsite and in secondary containment. The produced water tank containment contained a few inches of ice.

Some snow remained but areas around the pad were mainly bare. This site has mainly steep, rock lined slopes. Other areas around the pad and PCSM areas were greater than 70 percent vegetative cover. No E&S issues were evident during the inspection.

According to Department records the ESCGP for this site expired on 1/22/18. It is recommended that an NOT be submitted for review in the spring or summer if the site meets all restoration criteria.
