2324845 Primary Facility 2014-11-24 Routine/Complete Inspection No Violations Noted While in the process of surveying the containment and equipment and evaluation site conditions, I observed the evidence of a petroleum product release on the ground surface of the Pad at 41° 34’ 8”; 76° 13’ 9”. The pooled fluid on the pad surface was reddish/pink in color, indicative of diesel fuel. I also observed a rainbow colored sheen in a puddle of water nearby the pooled fluid. Sorbents pads used to control and contain the release remained in place. The location of the release and sheen observed was in the southeast section of the pad in between (2) mud pumps not stored on containment and the oil based mud tanks. the total amount of diesel fuel released to the pad surface for (2) separate releases was approximately (3) gallons.
2360120 Primary Facility 2015-04-02 Routine/Complete Inspection No Violations Noted There was no activity at the site during the inspection. The secondary containment used during hydraulic fracturing operations remains on-site. There are no frac tanks on-site. I recorded (2) milliSiemens/cm³ conductivity in pad surface water in front of the containment at N-41° 34' 10"; W-76° 13" 7'. Conductivity readings in water on containment were less than (1) ms/cm³. Conductivity readings ranged between (2-3.3) ms/cm³ in water at the end and below the diversion swale/rock-lined ditch below the fill slope of the pad at N- 41.57006°; W-076.21923° and in water just below a culvert in the diversion swale/rock-lined ditch at N- 41.56949°; W-076.21852°. Conductivity readings were less than (1) ms/cm³ in water accumulated next to filter sock below a rock spreader at in a rock-lined ditch at N-41.565958°; W-076.21847°. I also observed some trash on containment.
2360153 Primary Facility 2015-04-07 Follow-up Inspection No Violations Noted I recorded (1) milliSiemens/cm³ conductivity in pad surface water in front of the containment at N-41° 34' 10"; W-76° 13" 7'. Conductivity readings in water on containment were less than (1) ms/cm³. Conductivity readings ranged between (2-3.3) ms/cm³ in water at the end and below the diversion swale/rock-lined ditch below the fill slope of the pad at N- 41.57006°; W-076.21923° and in water just below a culvert in the diversion swale/rock-lined ditch at N- 41.56949°; W-076.21852°. I collected a water sample in the diversion swale at the first co-ordinates listed above in the inspection using the PADEP Bureau of Laboratories Standard Analysis Code 046. Conductivity readings were less than (1) ms/cm³ in water accumulated next to compost filter sock below a rock spreader in a rock-lined ditch near the access road and pad entrance at N-41.565958°; W-076.21847°, where I also collected a water sample using SAC 046.

FTS is performing the frack. No site security or check point.No leaks detected from equipment. Secondary containment nearly empty of all water.
2702813 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted Arrived at 1:30. Weather: cloudy threat of snow. Six wellson a six slot set up. Four wells are in production: 1H, 5H, 2H, and 6H. Two wells appear to be TD'ed but not completed. No violations were observed at the time of my inspection.
2814594 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:45; the weather was ~30°F and calm.

There were 4 active wells and 2 regulatory inactive wells onsite. All well cellars were filled with water; no bubbling was observed. There were also 4 GPUs, 4 sand separators, and 2 produce water tanks onsite and in secondary containment.

Overall, the site was greater than 70% vegetative cover. No E&S issues were observed. There were some sections of filter still around the pad (Photos #1, 2). There was also a section of hose on the pad surface that may or may not be needed for production (Photo #3).

According to Department records a restoration extension request was returned on 9/18/17 due PCSM calculations done for a reduced pad size, however the pad remained at its full size. It also appears that the ESCGP for this pad expired on 11/25/18.
