Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2332480 Primary Facility 2014-12-29 Routine/Complete Inspection No Violations Noted
2370977 Primary Facility 2015-05-19 Routine/Complete Inspection No Violations Noted Arrived on site at 11:25 and departed at 11:55. Weather conditions were sunny and ~80°F. Gary Blatten of Chesapeake was on site during my inspection.
2390195 Primary Facility 2015-07-02 Routine/Complete Inspection No Violations Noted Trican has completed hydraulic fracturing operations for the Makayla N Bra 1H Well as of yesterday, 7/1/15. Chesapeake was in the process of rigging down hydraulic fracturing operations. There were some tears observed in the containment liner in the area where the brine water tanks were stored near the site access road and site perimeter berm. Overall, containment at the site appeared to be in good condition. Water accumulated on the pad surface and containment did not reveal elevated conductivity readings of concern.
2391048 Primary Facility 2015-07-20 Routine/Complete Inspection No Violations Noted Completion activities have stopped. A workover rig is on site, and is estimated to be there for another 7-10 days.
2488011 Primary Facility 2016-05-27 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No Chesapeake representatives were present.

Site well vegetated with greater than 70% perennial vegetation. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. Electrical conductivity was not screened due to absence of puddles on pad.

No violations were cited during this inspection.

2564269 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted No violations cited.
2564270 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted No violations cited.
2679880 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted Five wells in production phase. Five each sand and water separators. No leaks detected from production equipment. No violations noted.
2801439 Primary Facility 2018-11-08 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:49; the weather was ~45°F and breezy.

There were 5 properly tagged Marcellus wells staged for production. No bubbling was observed in the well cellars. There were 5 GPUs, 5 sand separators, and 2 steel produced water tanks onsite and in secondary containment.

The site was greater than 70% vegetative cover where needed. Some of the steeper slopes were stabilized with rock lining. No E&S issues were noted. The pad appeared to remain at full size.

It appears that the ESCGP for this pad expired on 2/15/17 and a notice of termination (NOT) has not been submitted. It is recommended that if the site meets permitted restoration criteria an NOT be submitted for review or a restoration extension request submitted.

No violation noted.
