2690080 Primary Facility 2018-01-24 Drilling/Alteration No Violations Noted On January 24, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Scrivener Location in Rome Township, Bradford County. Andrew Hobbins was on site for the Department. Terry Hooper, Company Man, accompanied the Department during the inspection. Patterson-UTI rig 572 was on site and the drilling depth was 6200 feet.

Mr. Hooper stated that they are drilling a test Utica well to a total depth of 12207 feet. The last 250-500 feet will be core sampled so it can be analyzed. The 9 5/8 casing will be set at 8800 feet. There are no plans to drill horizontally at this time.

The Department walked around the pad and observed the containment liner. The liner appeared to be in good condition. The Department did not observed staining on the pad surface, but snowfall before and during the inspection made finding stains on the pad difficult. The cuttings are being solidified with a lime additive. The solidified cuttings are being sent to the Chemung Landfill in NY and the No

2693627 Primary Facility 2018-02-01 Drilling/Alteration No Violations Noted I arrived at the site at 11:29. The weather conditions were cloudy skies, light winds, with temperatures in the mid to upper 30's. I initially met with Chris Schmalz, Drilling Superintendent of CHK. Patterson Drilling Rig # 572 is drilling the Scrivener 35H well at a depth of (7,830) feet. Mr. Schmalz stated this is an exploratory (test) well and will be drilled vertically until core samples are collected in order to determine if the well will yield viable quantities of natural gas. If so, then the the horizontal section of the well will be drilled and the well would eventually be stimulated. I met with Terry Hooper, site company man, consultant of CHK and Mike Winters, Engineer out of the CHK Oklahoma City office in the company man trailer shortly after my conversation with Mr. Schmalz. Mr. Hooper concurred with Mr. Schmalz statements and explained them in further detail by stating the following. "This is an exploratory well. We will drill straight down and core with drill bit to pull out of pieces of the
