2701978 Primary Facility 2018-03-06 Preoperation Inspection No Violations Noted Site visit with contractors for contract 17-3 prebid meeting. A gas odor was present between casing and production tubing. Bubbling can be heard in the well.
2774602 Primary Facility 2018-09-13 Plugging(Includes Plugged/Mined Through) No Violations Noted Well is being plugged under contract OOGM 17-3R and is in a gas migration area. S&T Service and Supply (contractor) mobilized equipment on 9/10 and set up. On 9/11, they tried to take a reading of the well pressure but a 100# gauge was pegged. The well was blown down over several hours. On 9/12, they ran their downhole camera to a depth of 1575' (max for camera) and were still in 4". They said there are some noticeable holes in the 4".

Today they are running approximately 2056' of 1 1/2" in an attempt to find bottom. More 1 1/2" will be brought in as needed. After the 4" is pulled and casing seat plug is set, we will run a down hole camera in to look for holes in the casing.

The well continues to vent a high volume of gas. S&T (contractor) hauled in additional 1 1/2" tubing in an attempt to get to bottom. Pipe hit bottom at 2618'. S&T plans to set 30 sacks of cement at bottom, tag TOC, then pull 4".

2776365 Primary Facility 2018-09-18 Plugging(Includes Plugged/Mined Through) No Violations Noted Bottom plug tagged around 2400'. Plugging string removed and 4" locating tool run to determine amount of 4" in hole. Approximately 1900' of 4" is in hole. S&T is setting cement up to 1900', then will shoot the 4" off.
2779151 Primary Facility 2018-09-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Well being plugged under contract OOGM 17-3R.

1904' of 4" in well. 4" was shot at 1816 and would not budge. They shot again at 1663' and 4" was still stuck. A third shot at 1508' finally caused the 4" to break free. Approximately 600' still needs to be pulled.

1508' of 4" pulled. 30 sacks of cement inside and over where the 4" was shot. Plug tagged at 1291' yesterday. Casing seat plug tagged at 500' this morning. Well is being bailed out before topholing well. During bailing it was noticed that the water level is decreasing slowly, which would indicate a hole in the surface casing.
