At time of inspection no E&S issues observed on shared access road. Due to activity on site, Department unable to walk site to inspect temporary BMP's however, no E&S issues observed at this time. Department recommends Operator monitor and maintain BMP's to avoid any E&S issues. It appears well has been plugged. Two roll-off boxes on site. Plugging rig has been removed from site and Operator appears to be preparing to remove remaining equipment from site. As per current regulations, Operator has 30 calendar days from plugging a well to submit a certificate of plugging to Department. In addition, Operator has 9 months from cessation of earth disturbance activities to permanently stabilize the site.

2532943 Primary Facility 2016-10-31 Compliance Evaluation No Violations Noted Well was plugged by EXCO. EXCO subcontracted Kriebel to plug well. Well was wire lined and depth of well and casing intervals matched the original well completion report of 8880 feet total depth with Oriskany perforated zone at 8788 ft. to 8802 feet interval and surface pipe (504 ft.) and intermediate pipes (1500 ft.) cemented to surface. EXCO perforated at 300 foot intervals with 3 shot the length of the 5 ½ inch production pipe. EXCO cemented the production pipe in place by squeezing and filling the production pipe with 1160 sacks of Class H cement from 8,880 feet to 50 feet to surface where the remaining hole was filled with pea gravel.
