2792727 Primary Facility 2018-10-17 Routine/Partial Inspection No Violations Noted A notice of intent to plug for this well was received with work to begin on 10/23. The well had 2" inside of 6 1/4" casing, as the well record confirms. There was a drip near the well which was unhooked. The well did have a label and the coordinates on the NOI were accurate.
2800866 Primary Facility 2018-11-08 Routine/Partial Inspection No Violations Noted At the time of inspection, Howard Drilling has a rig set up over the well. No activity was taking place. A doghouse was on location as well. A pipe trailer and dozer were located along the access road where the road was more stable.
2804943 Primary Facility 2018-11-14 Routine/Partial Inspection No Violations Noted At the time of inspection, it does not appear that any activity has taken place since my last inspection. Pad and access road are very muddy.
2823693 Primary Facility 2019-01-02 Routine/Partial Inspection No Violations Noted At the time of inspection, well has been plugged. A monument was set above ground as a post with a plate welded on it which has the API number cut out of it.

There were 3 zones that are known to bear oil, gas, or water that were not cemented while plugging. Based on information gathered (surrounding wells that had logs) and my interpretation of the geology, there were 3 zones that were neglected. In the schematic attached, it shows the 3 zones at depths of 1610'-1630', 1710'-1730', and 2450'-2480' with the 2 shallower zones overlapping for cement according to the following regulation. Although the 2" production casing is cemented in, the regulations require cement to be set 50' below to 100' above any oil, gas, or water bearing zones [§78.95(B)].
