2717788 Primary Facility 2018-04-12 Routine/Complete Inspection No Violations Noted A routine, pre-plugging inspection of this deep vertical well was conducted April 12, 2018. At the time of inspection, cellar and surface casing are buried and only B section of wellhead with master valve on top is visible. The production casing wing valve on spool is tied into a production line. There was no venting, discharges, bubbling, or any obvious issues observed at this time. EQT has submitted an NOI to plug this well and plugging operations are tentatively scheduled to commence this spring.
2763629 Primary Facility 2018-08-14 Plugging(Includes Plugged/Mined Through) No Violations Noted CC Forbes' workover rig and crew are on location for final plug and abandonment of this Oriskany well. I met with Penn Mechanical, third party plugging contractor overseeing the job, to discuss operations on the pad. The well had 60 psi before moving in. They have just tagged bottom at 7300'. They intend to run gauge ring and set CIBP in the cased hole thereafter to isolate the productive formation. Productive casing above TOC is thought to be recoverable on this well. No obvious major issues were observed at this time
2766864 Primary Facility 2018-08-21 Plugging(Includes Plugged/Mined Through) No Violations Noted CC Forbes' workover rig and crew are on location for final plug and abandonment of this Oriskany well. I met with Penn Mechanical, third party plugging contractor overseeing the job, to discuss operations on the pad. CIBP has been set in production casing (7263'). Well was dead after CIBP was set. Casing was cut at 6123' and 6004' in attempt to retrieve production casing which was unsuccessful. The well now has a low volume of gas coming from the 7" casing annulus and a trickle of water from the casing. I reviewed the CBL; it appeared to show possible TOC at 5150' and some additional bonding shallow at 40'. Discussed potential contingency plans with the contractor in he event casing is unrecoverable. No obvious major issues were observed.
2766869 Primary Facility 2018-08-23 Plugging(Includes Plugged/Mined Through) No Violations Noted CC Forbes' workover rig and crew are on location for final plug and abandonment of this Oriskany well. I met with the Companyman and third party plugging contractor overseeing the job, to discuss operations on the pad. CIBP has been set in production casing (7263'). Well was dead after CIBP was set. Casing was cut at 6123' and 6004' in attempt to retrieve production casing which was unsuccessful. Gas has been observed in the 7" annulus. At the time of inspection; 7" production casing is still in the hole and the rig is circulating before pumping 400' cement plug back to approximately 5700'. From there they make another attempt at separating 7" at approximately 5100', above where CBL indicated possible TOC at 5150'. No obvious major issues were observed at this time.
2769920 Primary Facility 2018-08-30 Plugging(Includes Plugged/Mined Through) No Violations Noted The crew are rigging down following P&A. I spoke with plugging contractor to discuss operations since my last visit. After several attempts to recover production casing were unsuccessful, they plugged across all cuts and tagged cement at 4150' inside of 7". A squeeze attempt was executed across the casing shoe; perfs from 1359' to 1159'. The squeeze appears to be successful; they broke circulation behind the 7" and pumped shoe plug with excess -- calculated volume assuming 7" annulus was open. They WOC and tagged near calculated at 929' conveying TOC behind pipe is likely at that depth. Bailed hole dry as per coal regs. No gas reported. The well is shut in at time of inspection. They are preparing to install vent pipe as a well monument and vent both the 7" and 9 5/8" separately. No obvious issues were observed.
2772865 Primary Facility 2018-09-10 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged Oriskany well was conducted September 10, 2018. The rig is off location following plug and abandonment. At the time of inspection, two separate 2" vent pipes and marker plate have been installed on the plugged well as a plugged well monument. The vent pipe with the marker plate is for 7" production casing (unrecoverable) while the other vent is for 9 5/8" surface casing. The area around the well has standing water; no bubbling was observed. There was no audible or visible venting at either vent pipe. No gas was detected with an Altair 5x gas meter at either vent. No obvious issues were observed at this time.
2779949 Primary Facility 2018-09-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged Oriskany well was conducted September 26, 2018. 2782767 Primary Facility 2018-10-02 Administrative/File Review No Violations Noted October 2nd, 2018: Certificate of Well Plugging for EQT's State Tract 86 #591048 (033-20230) well has been received, reviewed, signed, and sent to file.
2856279 Primary Facility 2019-03-20 Site Restoration No Violations Noted Department representative Sue Herzing conducted a well site restoration inspection at EQT Prod Co. plugged conventional well 033-20230 on March 20, 2019. Al Reken of EQT assisted me during my inspection. The well was plugged in August 2018 in accordance with a CO&A EQT has with the Department.

The site has been restored back to approximate original contour. The site was seeded with DCNR's conservation seed mixture. Overall the restored site exceeds 70% vegetation.

Per this inspection I hereby approved the well site restoration report submitted for well 033-20230.
