Clearfield County conducted September 22, 2016 at approximately 12:00 pm. The well is off Sandy Ridge road. At the time of inspection, a service rig was rigged up on the well for plugging ops. I met with Matt from Fairman Corp. to discuss operations up to this point. Surface pressure on the well before operations was approx. 90 PSI. They killed the well with approx. 200 bbls of water. Wireline was on location to log the hole. CBL indicated TOC @ 1870’ on the backside of 4 ½” production csg. R&R Cementers are scheduled to be on location Monday to spot cement inside the 4 ½” from bottom, plug back to TOC on the annulus, WOC overnight, tag plug on Tuesday and cut csg. above plug so the rig can pull 4 ½” and spot remaining balanced plugs.
2522861 Primary Facility 2016-09-27 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s John Long (#1, 033-20435) conventional gas well in Bell Twp. Clearfield County conducted September 27, 2016 at approximately 11:00 am. At the time of inspection, a service rig was rigged up on the well for plugging ops, in the process of pulling 4 ½” production casing out of the hole. I met with Matt from Fairman Corp. to discuss operations since my last visit. R&R Cementers were on location yesterday where they plugged back from attainable bottom (3300’) by mixing and pumping 118 sks of Class A cement. The crew WOC 1 day and tagged the top of the plug this morning at 2042’ (top perfs at 2569’). Wireline had just pulled off location, made cut in the 4 ½” at 1854’. The rig was done pulling 4 1/2” by the time I left location at noon. The operator will call on cement crew to come out on location when available and plan to spot (1) single balanced plug from the 4 ½” csg stub, back through the openhole section and across the csg shoe 100’ up into the 8 5/8” csg (Top of plug @ 6
2526205 Primary Facility 2016-10-06 Plugging(Includes Plugged/Mined Through) No Violations Noted On October 6, 2016 at 1:15 pm I conducted an inspection of the John Long (033-20435) well in Bell Twp. Clearfield county following last week’s Plugging Operations. The rig and equipment are off location and 16”, 11 ¾”, and 8 5/8” casings remain cut at ground level. I placed a bucket over the 8 5/8” and rigged up my Altair 5x gas meter and monitored for 5 mins. No gas in any amount was detected at this time. Both the 8 5/8” csg’s annulus and the 11 ¾” csg’s annulus had water in them and there was no active bubbling at this time. I plan to return after the operator welds the monument and 2” candycane vent to verify readings.
2775136 Primary Facility 2018-09-11 Site Restoration No Violations Noted Restoration inspection on September 11, 2018 with Santo Adams of Fairman Corporation at restored conventional well 033-20435. The well has been plugged and the area surrounding the plugged well has been restored. The restored site is very well vegetated.

Per this inspection I approve the well site restoration report submitted for 033-20435.
