2682428 Primary Facility 2018-01-12 Incident- Response to Accident or Event Violation(s) Noted On 1/12/18 at 1028 hrs. I conducted an incident inspection in response to possible storage tank release at Diversified's 38244 well site (API 047-23841) in Ridgway, Elk Co. Upon inspection of the tank and surrounding areas, I discovered a small area of soil in the containment berm with conductivity over the range of my conductivity meter (over 20 mS/cm). The storage tank also appeared to have a small pinhole leak in the bottom. The tank had been disconnected and tipped on its side. The overall release was most likely less than a barrel of fluid, given the small amount of impacted soil. A visual cleanup/screened cleanup of the berm is required.

The release was unreported to the Department and constitutes violations of 91.33(a) and 78.66(a) which have been immediately resolved. Violations of 78.57(a), CSL 402(b), and SWMA 301 will be resolved upon cleanup of the release area and submission of disposal receipts to the Department.

2726323 Primary Facility 2018-05-04 Compliance Evaluation No Violations Noted On 5/4/18 at 0945 hrs., I conducted a compliance evaluation inspection of Diversified's 38244 well site (API 047-23841) in Ridgway Twp., Elk Co. Of particular concern was the status of the violations from inspection ID 2682428, which were cited due to a small brine release from the production fluids tank. On 4/26/18, I received an email notification from Kirk Elkin with Diversified stating that the cleanup had been completed for several months and that the production fluid tank had been repaired and re-connected for production. Upon inspection, the impacted soil from the containment berm had been removed and the berm was temporarily stabilized. The production fluids tank was repaired and re-connected for service. This inspection resolves violation ID numbers 805947, 805948, and 805949 from inspection ID 2682428.
