2634094 Primary Facility 2017-09-08 Routine/Complete Inspection No Violations Noted On Friday, September 8, 2017, I conducted a routine-complete inspection of the Pettit Well Site located in Washington Township Greene County.

The Act 9 sign and a separate sign with API permit numbers were in place a clearly legible at the site entrance. All required documentation was available on-site.

The access road and well pad and impoundment were in good condition. Overall, the site is well vegetated and exhibits uniform perennial growth throughout the majority of the site. There are a number of area with less than uniform growth. Some areas around the impoundment appear to have been worked relatively recently which would explain the lack of growth. Other areas around the site appear to have bedrock outcropping which will definitely not support growth. The might consider re-seeding areas of minimal growth with the exception of those areas with bedrock outcropping.

The secondary containment structures on-site appear to be in good condition and functioning as intended. The gas wells appear to b
