2511696 Primary Facility 2016-07-26 Routine/Complete Inspection No Violations Noted

On 07/26/16 at approximately 1330 hours I conducted an incident response inspection at the EQT Hildebrand well pad in response to a BMP blowout at the site. The Act 9 sign was in place and all required documents were present.

The site was being restored at the time of the BMP failure. A large rainfall event had occurred, overwhelming the sediment trap and causing the downslope wall of the sediment trap to fail. The resulting water, rock, and sediment broke through multiple silt socks leaving the LOD and entering a stream. A large amount of rock and gravel was found in the stream bed approximately 150’ from the sediment trap failure (see photographs). A pond approximately 200-250’ downstream was discolored from the sediment released from the BMP failure. At the time of inspection an excavator was repairing the breach in the sediment trap wall. EQT was advised to apply for an emergency 105 permit to repair the section of the stream that was impacted by the BMP failure.

2547890 Primary Facility 2016-10-14 Compliance Evaluation No Violations Noted

On 10/14/16 at approximately 1100 hours I conducted a compliance evaluation inspection at the EQT Hildebrand. The Act 9 sign was in place and all required documents were present at the site. In July 2016 the site had a sediment trap blowout, resulting in rock and sediment breaking through multiple silt socks and impacting a nearby stream. An emergency 105 permit was obtained by EQT and the material was removed from the stream. The sediment trap and erosion had been repaired at the time of inspection and the site was exhibiting new vegetation. There were no violations observed at the time of inspection.

2569666 Primary Facility 2017-03-07 Complaint Inspection No Violations Noted

On 03/06/16 a complaint was received by the Department alleging BMP failures at the Hildebrand well site. On 03/07/17 at approximately 1330 hours I, along with Water Quality Specialist Supervisor John Carson, conducted a complaint inspection of the site. The Act 9 sign was in place and all required documents were present.

Gravel from the site was found across Bowser Road. This is a violation of 102.5(m)(4) for impacts outside of the limits of disturbance.

A black corrugated pipe that was not shown on the E&S plans was observed on the right side of the access road immediately upon entry to the site. It is requested that EQT provide information on this pipe in the NOV response.

The rain garden was not functioning as designed and is at risk of failure. Water was bypassing the riser structure and draining through the soils between the rain garden toward the channel near the stilling well. A significant amount of sediment was observed in the rain garden and sediment laden water was observed in the stilling

2595660 Primary Facility 2017-04-05 Follow-up Inspection No Violations Noted On 04/05/17 at approximately 1200 hours I along with WQSS John Carson and Environmental Engineer Donna Davis conducted a follow up inspection of the Hildebrand site in response to an NOV issued on 03/07/17.

The leak around the riser structure in the rain garden had been repaired. The rain garden appears to contain sediment from poorly stabilized adjacent slopes. The slopes appeared to be failing, depositing sediment into the rain garden and causing a loss of volume. Additionally, erosion was occurring along the southwestern rip rap channel into the rain garden. It is recommended that this be repaired to prevent further erosion and volume loss. It is requested that EQT contact Donna Davis to discuss maintenance of the BMP. It is requested that EQT contact Donna Davis regarding the cross drain causing erosion into the pond on UNT to Castile Run.

The silt socks that were previously undercut or overtopped had been maintained to prevent undercutting, but sediment was still close to or at the top of the silt soc

2638145 Primary Facility 2017-09-20 Site Restoration No Violations Noted A site inspection was conducted in response to EQT requesting a meeting to discuss site vegetation and silt sock removal. The Act 9 sign was in place and all required documents were present at the site. The site overall was well vegetated. The erosion channel from near the access road to the location of the removed sediment trap was vegetated. It is recommended that the erosion channel be repaired to original grade, seeded, and mulched. The rain garden appeared to be in good condition. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2692715 Primary Facility 2018-01-29 Site Restoration No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site entrance exhibited signs of access road erosion (see photographs). It appeared water was overflowing the culvert and removing material from the access road near the Act 9 sign.

The site did not exhibit uniform 70% vegetative cover. Areas of poor or no vegetative cover were observed. Erosion was occurring along the access road below the check dams in the location of the removed sediment trap (see photographs).

The site does not meet the requirements for permit termination. The NOT is denied.

2760152 Primary Facility 2018-07-19 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal from EQT Production Co. The Act 9 sign was in place and all required documents were present.

Erosion of the access road was observed at the site entrance. Metal grates were in place over a large channel in the access road (see photographs). The channel continued off the site parallel to Bowser Road heading South. Erosion of the access road along the right side was also observed (see photographs). Erosion was observed adjacent to the left side of the access road below a neighboring pond, exposing a black pipe.

Poorly vegetated soil and accelerated erosion were observed where silt sock was removed in the area of the restored sediment trap. Multiple small erosion rills were observed in the vegetated area upslope. It appeared the rills existed and were not repaired prior to the vegetation grew and covered the rills. It is recommended the accelerated erosion is repaired.

2809722 Primary Facility 2018-11-14 Site Restoration No Violations Noted A meeting was requested by EQT to evaluate the site for NOT criteria. The Act 9 sign was in place and all required documents were present at the site. PCSM was installed and appeared to be functioning. Site vegetation met the requirement of 70% uniform vegetative cover. The site meets the requirements for permit termination. The NOT is approved.
2849648 Primary Facility 2019-02-25 Routine/Complete Inspection No Violations Noted On 02/25/19 at approximately 1220 hours I conducted an inspection of the site. It appeared recent heavy rains had caused the pit collecting water for the culvert to overflow at the site entrance, causing wash out of the access road. Ice was present on Bowser road and appeared to have been caused by water leaving the well site.

Water from the adjacent private pond was eroding through the pond bank along the pipe leading to the culvert inlet. The inlet from the pond was clogged with debris, causing water to divert around the pipe and erode alongside it.

The culvert appeared to be functioning at the time of inspection.
