2480588 Primary Facility 2016-04-26 Routine/Partial Inspection In Compliance with Policy On-site meeting as requested by Operator representative Kenny Radzieta to review E&S plans for the transfer of a gathering pipeline. XTO representative Gordon Bish and Mountain Gathering representative Jerry also present.

According to Mr. Radzieta, PENNDOT is replacing a bridge where XTO’s gathering pipeline is installed under the stream that this bridge crosses. In order for the bridge replacement to be completed, the gathering pipeline must be removed. The Operator is planning to replace this gathering pipeline a short distance from its former location and intends to underground bore for the pipeline installation. The gathering pipeline is to be installed under an agricultural field. Mr. Radzieta inquired as to whether a temporary road is necessary for the work required for this project. As the Operator has hired a consultant to develop an E&S plan for this project, the Department recommended that the Operator implement the E&S plan as outlined in accordance with current regulations. The representatives

2563245 Primary Facility 2017-02-07 Routine/Complete Inspection In Compliance with Policy On-site meeting as requested by Operator representative Marc Maestra to review E&S plans for the transfer of a gathering pipeline. Operator representative Mike Purdom and Department WQSS April Weiland also present.

Per Mr. Purdom, the Operator intends to underground bore for the installation of the new pipeline. The gathering pipeline is to be installed under an agricultural field and one stream. The bore is to occur 30 feet under the stream. The Department reviewed the E&S plan for the project. The Department recommends that additional temporary E&S BMPs are installed as necessary in order to prevent any potential pollution events from occurring. The Operator shall install the temporary E&S BMPs prior to conducting any earth disturbance. The Operator anticipates commencing this pipeline bore within the week and will notify the Department prior to doing so. Operator appears to be in compliance with current policy at this time.

2563309 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted At time of inspection, operations for underground bore in process. Operator representative Mike Purdom and contractors on-site.

E&S plan on-site. Temporary E&S BMPs appear to be installed in accordance with E&S plan. No accelerated erosion and/or sedimentation issues observed during this inspection. Per Mr. Purdom: Boring operations commenced yesterday (2/14/17). At this time, the bore is on the threshold of crossing underneath the stream. There have been no issues with the bore to date. Mr. Purdom will notify the Department of any issues and/or concerns that may arise with this project and will notify the Department when boring operations have been completed. WQSS A.
