IT WAS REPORTED THAT GAS WAS RELEASED AND SPRAYED THE WATER OUT OF THE WELL. NO WATER WAYS REPORTED TO BE AFFECTED. SPILL REPORTED TO BE CONTAINED AND CONTRACTOR TO TEST THE WATER TOMORROW.

2522088 Primary Facility 2016-09-22 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to admin inspection 2522066 for the reported release of “7 bbl gas”. At 0714, the Department contacted Operator representative Kyle Borden (Environmental Compliance Specialist) upon receipt of the spill notification.Per Mr. Borden, the well, which is actively being plugged, was loaded with approximately 20bbl freshwater and was perforated at 150 feet. There was a kick of gas that caused water to blow out of the top of the hole as a fine mist. The Operator believes it is freshwater because the hole was reportedly dry before the freshwater was added. Mr. Borden stated that there may have been some cement in the water as cement was being added during this process.Mr. Borden reported a release of 7 bbl water based on the dimensions of the hole and calculations for water volume unaccounted for and the area of impact.However, the Operator believes that the release may be closer to 1-2 bbl water.Mr. Borden stated that there was no pooling of water and the water was immediately
2522122 Primary Facility 2016-09-22 Incident- Response to Accident or Event No Violations Noted On-site inspection conducted in response to admin inspection ID 2522066 for the 9/21/16 reportable release of 7 bbl water to the ground.At time of inspection, Operator representative Adam Shutler and plugging contractors on-site. Well actively being plugged.According to Mr. Shutler, a kick of gas during the plugging process caused freshwater to spray out of the well yesterday (9/21) at approximately 1600. It is believed to be freshwater as only freshwater was added to the well prior to the reported release. An area to the northwest of the well approximately 55’x55’ in size appears to have been impacted by the reported release of water. The fluid appears brownish/gray and appears to coat the vegetation. Per Mr. Shutler, the water is primarily mixed with dirt and bentonite. It is possible that the fluid also contains brine and/or cement.MSDS sheets for bentonite were on location. The Department was unable to locate an on-site E&S plan. The Department’s regulations provide that an E&S plan shall be developed, implemented, and present at the project site during all stages of an earth disturbance activity if the earth disturbance activity will result in a total earth disturbance of 5,000 square feet or more and/or if the earth disturbance activity has the potential to discharge to a water classified as a High Quality or Exceptional Value water under Chapter 93. Please note that this well site is located in a High Quality watershed.

Requested written response received by the Department on 1/3/2017. Per the Operator’s response, the E&S plan is now available on-site. Violations remain outstanding at this time.

At the time of this inspection, the Department is unable to locate the on-site E&S plan. Per the Operator's written response to the violations received by the Department on 1/3/17 (inspection ID 2550001), the E&S plan was made available on-site. While on-site, the Department contacted Operator representative Kyle Borden via phone to verify the location of the E&S plan. While talking with Mr. Borden, the Department was still unable to locate the E&S plan on-site. Mr. Borden stated that he would determine whether the E&S plan had been placed on-site and would follow-up with the Department. Temporary E&S controls installed along down-gradient perimeter of site. Site re-graded, seeded and mulched, nearing 70% uniform and adequate vegetative growth.

Voicemail received from Operator representative Kyle Borden on 1/4/17 at 1554 notifying the Department that the E&S plan is not yet on-site, but will be placed on-site soon. Mr. Borden will notify the Department once this is completed. Violations remain outstanding under inspection ID 2543314 (enf ID 349311).
