2369954 Primary Facility 2015-05-13 Incident- Response to Accident or Event Violation(s) Noted On-site inspt in response to admin inspt 2369909 for the 5/6/15 reportable release of 3.5 bbl crude oil/brine water to ground.At time of inspt, BT disconnected from well.WT present, legible.Drilling occurred 1/09/2009-1/15/2009.Faint gas odor noted.Hissing noise observed in well head vicinity.Area of excavation not yet backfilled, extends down-gradient of site.All visibly impacted materials appear removed.Booms in place down-gradient of excavation.Two covered roll-off boxes on-site.Shared AR stabilized.Dept recom'ds Opr install temp BMPs down-gradient of excavated areas as necessary in order to prevent any potential pollution events from occurring.A FUI will be conducted in the future to ensure site is properly remediated and restored.Viols issued under 78.54.Following inspection at 1034, Department notified Operator representative Jason Canose via phone of above-noted observations (i.e. smell, noise). Mr. Canose said that the Operator is aware and is in the process of resolving the issue.

Roll-off boxes no longer on-site.Shared AR stabilized.Upon further investigation into incident by Opr, Opr believes release was due to act of vandalism and calcs were submitted to Dept documenting release to be 41 gals as it was originally reported to be 3.5 bbl.A FUI will be conducted to ensure site is properly restored.Written response to viols received on 5/22/15.As all visibly impacted materials appear excavated and written response to viols have been received, viols issued under record 2369954 (enf ID 323220) have been corrected.
