1781551 Primary Facility 2009-01-23 Routine/Complete Inspection No Violations Noted Well-site not constructed at time of inspection. Inspection performed as a result of a waiver review due to the proximity of the well-site to an Unnamed Tributary of Crooked Creek. The Following deficiencies were found with the E&S plan and should be addressed before the waiver is approved: 1- Due to the probable high ground water table, below grade pits will not be able to be utilized on the well site, and the cuttings will not be able to be encapsulated. To land farm the cuttings, the seasonal high water table must be a minimum of 20" from the surface. If this criteria can not be met, the cuttings must be hauled to an approved landfill. 2- The well site is within the FEMA 100 year flood plain. Please submit a cross-section of the proposed final restored well site. Include any equipment that will be left on-site and mark the well-head protection barriers that will be installed to protect the well head. . 3- If possible, place all equipment outside of the flood plain. 4- Verify that a GP-7 is not required for the unnamed tributary.
1781555 Primary Facility 2009-04-08 Administrative/File Review No Violations Noted 3/27- the E&S plan revisions were received and it was found to be sufficient. If the E&S controls are properly implemented as discussed in the revised plan then there should be no impact to Crooked Creek.
