The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The access road and associated controls were installed and well maintained. The freshwater impoundment was installed and partially fenced. The pad surface was constructed and partially lined. The perimeter BMPs were installed and maintained. All slopes were stabilized. 2464352 Primary Facility 2016-03-08 Routine/Complete Inspection No Violations Noted The pad size measures ~300’x 350’. The main liner is still visible, along with smaller pieces of liner collecting rainwater around the perimeter of the pad. Puddles were observed on the east and west corners of the pad with conductivity measuring ~250 µs/cm. Three wells were visible on the pad. Each well/cellar has been covered by a large steel plate. The cellars appeared full of water. No well ID tags were visible. Bubbling was observed in the cellars of both the middle and eastern wells on the pad. The OGI inspector supervisor was notified. The outslopes of the pad surface appear stable, mostly rocky with patches of vegetation. Silt sock was observed around the toe of the slope.

An application to terminate a portion of the ESCGP-1 permit and re-permit the site under an ESCGP-2 was received by the NCRO on 9/28/15. This site is now permitted under an ESCGP-2, permit# ESX10-105-0013(02) No violations are issued at this time. Please contact me with any questions. I departed the site @ 1330.

2619153 Primary Facility 2017-07-19 Routine/Complete Inspection No Violations Noted On 7/19/17 at 1230 hrs I conducted an inspection at the Triana Young Pad A well site. No one was present during my inspection. It was mostly sunny and 83 degrees F.

The site consists of a well pad, impoundment and access road. It was originally permitted by an ESCGP-1 on 7/30/10; the permit was amended on 11/5/10 for the construction of the impoundment. On 12/14/15, the permit was amended again upon issuance of an ESCGP-2.

The freshwater impoundment is located along the access road, before the well pad. The impoundment is protected with a fence, had adequate freeboard and no holes were observed in the liner (Photo #4).

The 2H, 4H and 6H wells were drilled in 2011 and placed on inactive status on 1/16/14 (Photo #1). No new well permits have been issued for the location. The cellars were filled with water and bubbling was noted around the 2H and 6H wellheads. This information was passed along to the Oil and Gas Inspector Supervisor. 2627439 Primary Facility 2017-08-17 Compliance Evaluation No Violations Noted On 8/17/17 at 0914 hrs I conducted an inspection at the Triana Young Pad A well site. No one was present during my inspection. It was mostly sunny and 62 degrees F.

The purpose of the inspection was to follow up on violations cited against the 2H permit (105-21697) during the 7/19/17 inspection. Documentation of routine E&S inspections and a copy of the ESCGP-2 were not found in the site mailbox during the 7/19/17 inspection. Triana provided copies of the routine E&S inspections upon request, demonstrating that they are being completed. The violation was resolved upon receipt of the inspection documentation. In addition, old permits and paperwork were removed from the site mailbox and replaced with copies of the E&S inspections and the ESCGP-2. A lock was placed on the mailbox to prevent theft of the contents.

It was also noted during the last inspection that the well site did not appear to be restored and post construction stormwater management (PCSM) BMPs installed. In response, Triana's consultant prov

2628810 Primary Facility 2017-08-24 Administrative/File Review No Violations Noted On 8/24/17 I reviewed and acknowledged the Triana Young Pad A Well Site Restoration Report (WSRR). The report was submitted electronically on 8/22/17.

Site conditions were verified during inspections conducted on 7/19/17 and 8/17/17; the associated inspection reports document that post construction stormwater management practices are installed and the site is permanently stabilized.

No violations are cited in association with this administrative inspection. TLT

2640198 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted This well is in Regulatory Inactive Status at the time of this inspection. No company reps were at this inspection. Well area and well pad are neat and clean. This well is being vented into a small tank. It is being vented at the 9 5/8" x 5 1/2".
2838177 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells status was Regulatory Inactive Status. This inspection is for a status extension. At the time of this inspection this well has been drilled to total depth in 2011 but was not completed. This wells conductor, surface casing, intermediate and production strings (casings) are all vented together ( commingled) and are vented to a poly tank at the end of the row of wells. This poly tank has a vent and the vent was tested for combustible gas with an Altair 5 X Multi - Gas Meter that was calibrated on 2-4-19. The results of this testing was 100% LEL (lower explosion limit) and 45% METHANE. No violations were noted at this inspection.
