1995173 Primary Facility 2011-08-11 Compliance Evaluation No Violations Noted On August 11, 2011 a compliance evaluation was conducted at the Beech Flats 2H well site. The E&S plan, E&S controls and perimeter of the site were observed. At the time of the inspection, no activity was occuring on the pad. The road surface was being compacted. The surface of the access road has been reinforced with stone and culvert pipes installed. The outlets of the culvert pipes were installed according to the standard detail drawing in the E&S plan. The cut slope of the access road, log landing and topsoils stockpile has been stabilized with erosion control matting and was seeded and mulched. The seeding has not achieved 70% cover at the time of the inspection. A representative of Chesapeake indicated their plan is to removed the erosion control matting and hydoseed the cut slopes and log landing once they are able to resume activities on the site. Silt socks were present at the perimeter of the site and were installed according to the standard detail drawing in the E&S plan.
2004110 Primary Facility 2011-09-19 Routine/Complete Inspection No Violations Noted The E&S plan, E&S controls and perimeter of the site were observed. At the time of the inspection, the E&S controls were being completed, hydro-seeding was underway and the well pad surface was being constructed. The erosion control matting along the access road has been removed and the cut slopes hydro-seed. The topsoil stockpile had been recently hydro-seeded along with other areas adjacent to the well pad. The perimeter BMPs were installed and functioning. The compost sock sediment trap on the southeast side of the site was installed and functioning, but in need of maintenance. The silt in this trap should be removed to allow the BMP to function as intended. No violations were cited during this inspection. MRN
2007355 Primary Facility 2011-09-30 Complaint Inspection No Violations Noted An inspection was conducted at the Beech Flats pad on 9/30/2011 at 1330 in response to a complaint. Employees of the Galeton Borough, operators of the water supply, informed the Department that on Tuesday 9/27/11 or Wednesday 9/28/11 that sediment was leaving the access road following a heavy rain. The Borough contacted Chesapeake Energy to make them aware of the situation. Representatives of Chesapeake Energy responded the same day and observed that the ditch adjacent to the access road was not capturing all of the runoff. They immediately corrected the situation and no further sediment transport was observed.

A 5H well is permitted, but not yet drilled and the 2H well has been drilled. The access road is stable and the slopes adjacent to the access road are partially vegetated. The slopes adjacent to the pad are partially vegetated; the site is less than 70% vegetated at this time. E&S controls are installed, functioning and well maintained. No erosion was evident during this inspection. No new violations were cited during this inspection.

2573217 Primary Facility 2017-03-13 Routine/Partial Inspection No Violations Noted On 3/13/17 at 1420 hrs I conducted a routine inspection of the Beech Flats well site. It was overcast and 31 degrees F. No one was present during the inspection.

The well site was idle. The 2H well was plugged in July 2016 and no other valid well permits currently exist for the site. The well pad has not yet been restored. As per 25 Pa Code Section 78a.65(a)(2), within 9 months after plugging the final well on the well site, the operator shall remove all production or storage facilities, supplies and equipment and restore the well site. Based upon when the well was plugged, the well site should be restored by April 2017. If the operator is unable to restore the site, then a restoration extension request should be submitted to the Department as per 58 Pa. C.S. Section 3216(g): Well Site Restoration - Extension. The request should be submitted electronically on forms provided by the Department through the Department’s website.
