The E&S plan, E&S controls and perimeter of the site were observed. The access road and associated rock lined ditches were installed. The cut and fill slopes adjacent to the road and pad were vegetated and stabilized. The pad was constructed and stable with three cellars installed at the time of this inspection. All erosion control BMPs specified on the plans appeared to be installed. No violations were cited during this inspection. MRN

2044692 Primary Facility 2012-02-21 Follow-up Inspection Pending On Feb. 21, 2012 an inspection was conducted at the Arch Pot location in Sweden Township, Potter County. The purpose of this visit was to follow up on an inspection conducted on Feb. 17, 2012 were several issues resulting from poor housekeeping were observed. Samples of an unknown liquid were collected previously by another inspector from the pad surface and submitted for analysis. Violations are pending the results of this analysis. The issues noted in this inspections and the inspection conducted the Department on Feb. 17, 2012 indicate poor housekeeping procedures. The Department recommends you review your procedures regarding washing equipment prior to transporting it off-site to ensure it is in compliance with existing regulations. It is also a recommendation you review the protection the liner provides under the activity it is subjected to.

2165825 Primary Facility 2013-05-13 Routine/Partial Inspection No Violations Noted An inpsection of the Arch POT pad in Sweden Township, Potter Co. was conducted on May 13, 2013. A 4H well is permitted , but not yet drilled and the 5H well has been drilled. The access road is stable and the slopes adjacent to the acces road are well vegetated. The slopes adjacent to the pad are well vegetated; the site is more than 70% vegetated at this time. No new violations were cited during this inspection.

2642821 Primary Facility 2017-10-02 Routine/Complete Inspection No Violations Noted On 10/2/17 at 1319 hrs I conducted a routine inspection of the Arch well site. It was sunny and 70 degrees F. No one was present during the inspection.

The site, which consists of the access road and a 300' x 400' well pad, was idle. The 5H well has been drilled and was placed on inactive status on 3/26/14.

An ESCGP-2 (ESG29-105-17-0007) was issued on 7/27/17; this replaces the original ESCGP-1 (ESX11-105-0006), issued on 7/5/11, which authorized construction of the existing well pad. Chesapeake submitted a two-year extension request to the restoration requirements on 6/20/16.

E&S controls are in place and the site is permanently stabilized. New flagging, stakes and silt sock were present in locations where infiltration BMPs are to be installed. In total, six infiltration areas will be constructed-four along the access road and two adjacent to the well pad.

No violations were cited during this inspection. I concluded the inspection at 1505 hrs. TLT

2843599 Primary Facility 2019-02-22 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production. The well is in Regulatory Inactive Status and this inspection was done because of a Regulatory Inactive Status extension application requested by the operator. Well has two (2) annulus vents that are vented to surface and are identified by tags indicating the annulus size. A gauge on the top of the well head assembly indicated a psi (pounds per square inch) of 0. Each annulus vent also has a pressure gauge on the vent and both gauges indicated a pressure of 0. No violations noted at this inspection.
