2705622 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. All annulus vents , flow lines, and fittings were tested for gas . Results of this testing was that no gas was detected on the wellhead assembly or the well area.
2721123 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted Minimal activity on site when I arrived. I met Sean Bacha, production consultant for JKLM. AST present on the site, filled to within 8-10' of the top. Secondary tank contained little to no water. Puddles on the pad and around the tank showed conductance from .19-.38 mS/cm. Puddles immediately in front of the offloading area for the tank were slightly higher (.85-2.15 mS/cm). A bank of Baker tanks was also present on the site; Sean informed me that the produced water from the pad had been going to SWEPI, but at present they are not taking any. Overall, site looked good; however, I was unable to locate any well tags on the wells. I recommend that tags be affixed ASAP, or if they were present, that they be made more visible and obvious.
2742322 Primary Facility 2018-06-14 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the JKLM – Reese Hollow 117 on 6/14/2018 @1245 hours. The weather was 70 degrees, 38% humidity, winds WNW 20 mph. The well site is currently in production.

The site was last inspected by company personnel on 6/14/2018 with the comment “elevated conductivity readings recorded in two areas outside of containment.”

A puddle was observed along the edge of containment in front of the AST load/offload area. The puddle had a field measured conductivity of 2.9 -3.5 mS/cm (see photo #2). Fluid inside the larger containment was measured at 9.04 mS/cm (photo #3) and “Or” in the smaller duck pond containment (photo #4 and #5). A smaller duck pond containment was laying adjacent to the AST with a field measured conductivity of “Or” or over range (photo #7).

A puddle was observed on the pad surface in front of the tank battery with a field measured conductivity of 3.73mS/cm (photo #9 and #10). A vac truck driver was located onsite. I informed him of the areas with elevated cond

2746432 Primary Facility 2018-06-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. No violations were noted at the time of this inspection.
2765801 Primary Facility 2018-08-20 Routine/Complete Inspection No Violations Noted I arrived on the site and met Sean Bacha, consultant for JKLM. Little activity was occurring on the pad. Puddles around the offloading area for the MASS showed low conductance (.53-.75 mS/cm); water in the containment itself was slightly higher (.87 mS/cm), and was within 2-3" of the top of the berm. A vac truck was already on location, and Sean informed me that it would be vaccing this containment. The MASS was fairly full, with approximately 3-3 1/2' of freeboard, and the secondary containment had no standing water. On the remainder of the pad, all puddles showed low conductance (.33-.68 mS/cm).
2768248 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. A digital gauge on the 3" production line leaving the well head assembly and going to the production equipment read 1039 psi (pounds per square inch) at this inspection. No violations noted at this inspection.
2803135 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. There are four (4) annulus vents openly venting on this well. Annulus vent surface x intermediate (20" x 13 3/8") was noted venting 100% LEL (lower explosion limit) and 7% METHANE at this inspection. No other annulus vents were noted venting at this inspection. Combustible gas detected at this annulus indicates potential defective cement. JKLM should monitor this annulus on a monthly basis and report any significant changes to the Department as soon as possible. The Department will continue to monitor all annulus vents at this well during future inspections. No violations noted at the time of this inspection.
2814173 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Reese Hollow 117 Well pad located in Sweden Twp, Potter Co on 12/11/2018 @1245 hours. The weather was 25 degrees, parlty sunny, 2-4 inches of snow on the ground. Winds WSW @10mph, 68% humidity. Sean Bacha, JKLM representative was present onsite during the inspection.

The well site appears to be setup for production. Four wells are listed as in "active" status. The Aboveground Storage Tank (AST) appears to have been removed. A large pile of gravel was observed in it's place. Mr. Bacha stated the waste containers staged onsite are from the removal of the AST.

The secondary containment around the production tanks and the tank battery were inspected. No staining or puddles were observed on the ground surface. No fluid was observed in the secondary containment.

The onsite E&S inspection log states the site was last inspected on 12/7/2018.

No violations are noted at this time. I departed the site at 1415.
